Request Reports or Records
Arvada Fire is dedicated to being an open, transparent public service agency and upholds the public's right to request records from the fire district. To expedite your request, be as specific as possible in describing the records being requested. Please follow the tabbed list below to find the appropriate form for your request.
Protected Information
Do not use these forms to request Health Insurance Portability and Accountability Act (HIPAA) protected information. Due to HIPAA regulations regarding patient confidentiality, requests for patient care/emergency medical reports must be submitted directly to APEX Billing by calling 720-486-0622.
A Fire Incident Report is a National Fire Incident Reporting System (NFIRS) compliant report from our operational crews responding to an incident.
Requests are processed weekly on Fridays and will be sent via the method requested.
Due to HIPAA regulations regarding patient confidentiality, requests for patient care or emergency medical reports must be submitted directly to APEX Billing.
A variety of record request types — such as Environmental Site Assessments, fire investigation reports, and fire inspection reports — are available through general request.
When filling out the form, please select "General Inquiry Only" under the Request Type field. To expedite all requests, please be as specific as possible in describing the records being requested.
If you wish to receive only an Incident Report, please use the Incident Report form.
Citizens have the right to request information about government and public agencies like Arvada Fire because of open records laws. The Colorado Open Records Act "CORA" requires that most public records be available to the public.
To file a CORA request, please select "Official CORA" under Request Type within the form. For general requests, you can select "General Inquiry Only". To request only an incident report, please use the incident report request form.
